Youth Exchange - Both Rotarians and Host Families must follow these guidelines to participate in Youth Exchange

Instructions are contained in all the documents. Please use the fillable forms if possible. Other fillable formats are being explored.

Confidentiality - Only two people process consent forms, Tom Port for Youth activities other than Youth Exchange, and Michael Bury for Youth Exchange.  The authorization forms are MAILED by the applicant directly to the applicable person. Both are attorneys and are as such obligated to privacy and security practices.  No one else in Rotary has access to the consent form.  The SSN’s are guarded and kept in secure files. Applications are processed on line through a secure service center..

All YEO’s, counselors and club members expecting to have one-on-one contact with a YE student during the year are to submit a Volunteer Affidavit Form (VAF) plus an Authorization Consent Form (LS).

All host families are to submit one Host Family Application (HFA) plus an Authorization Consent Form (LS) for each residing family member 18 years of age or older.
An LS submitted last year is valid for this year… no need to repeat.  If someone doesn’t recall, ask them to go ahead and submit.

The Notice of Student Arrival (NSA) should be submitted by the counselor picking up at the airport.

Student Change of Address - Please fax or email this form preferably before your inbound student’s move, but no later than 36 hours afterwards.
